Membership Application Form: 1st April 2012 – 31st March 2013
Please complete the 2nd page of the application form (click on the Word Icon at bottom of page) following the instructions below :-
1.
A cheque for £45 ( no cash ) payable to BA Clubs, include on the back your BA Club Number and state Golf Section and for:
i, Staff members: STAFF NUMBER, WORK ADDRESS
ii, Non Staff members: BANK CARD NUMBER with expiry date
2. A self addressed envelope with stamp.
3. New members should supply a copy of their existing handicap certificate or 3 completed and signed scorecards. These can be supplied later but a BA Golf handicap cannot be issued until the scorecards are received.
4. No Competition or Day Meeting will be allowed until the handicap certificate or 3 cards have been passed to the Handicap Secretary.
5. Please note BA Clubs membership must be current, if your BA clubs membership expires you cannot take part in section activities and membership of the Golf Section will also expire.
